Your First Visit

1. Call or send us a message. We'll talk through what brings you in and find the right first appointment.
2. Complete your forms. Customized care starts with a complete history, so please finish your forms at least 48 business hours before your visit. Without them, we may need to reschedule.
3. Come in and tell us your story. Your first visit is about listening, a thorough evaluation, and a clear conversation about options.

• Deposits. We hold your appointment time just for you, so new appointments require a deposit, much like a hotel or rental car reservation.
• An exam comes first. A dental examination is needed before specialized appliance therapy begins.
• PWA agreement. NeuralFORM Dental Device Therapies and non-surgical laser therapies require our Professional Wellness Alliance Membership Agreement.
• We're your dental home. As a general dental office providing specialized services, we take responsibility for your overall dental health, not just one treatment.

We work with most dental insurance plans and are glad to help you understand your benefits. No dental insurance? Our in-office SmileSavers plan may be a good fit.
